How to get Kuugai Title ?

It's referring to getting the STAFF to enhance levels, not player - item level. You have to get the staff to +60 to unlock the title.

So get to farming. Try starting at Den of Needles in Seki.

its only about possession, so yes, you can get your title by buying a pre-enhanced staff. there is no need to grind the staff and enhance it by yourself.
