I just reached Sadoshima. If I come back to Echigo without clearing Sado Mine, will I have to clear the undersea cave again to go back to Sadoshima?

after clearing Sado Mine a bird with quest will appear at the entrance to Sadoshima, the quest is to give 2 Yagyou-san's Tears, when you do it the bird will appear in Echigo a bit before Undersea Cave entrance, and the shortcut to Sadoshima behind it

otherwise you'll have to clear undersea cave everytime

You can only succeed one title at a time.
I don't get your 2nd question. Each title has it's own stats. Some give affinity, some give stats, some give effects, and some have a combination of all. Refer to the wiki if you want to see the list of titles.

Not in any quest afaik (pretty much done every quest -(a few edo ones) -.-). So yeah, I guess you have to grind it...

It seems like something like a weapon specialization, but can't say for sure as I don't believe there's any information available on it at the moment.

For now, I'd consider it hope that CS still has plans for this game.
