You're not using the right skills, refer to the wiki.

If you expand a skill, it will list its effective range. Assuming enemies don't walk away from you, most skills should have about 14m. Note that most of the ranges are extracted from the game files, so they're definately accurate. While it is about luck, you can always reroll them with skill change. Also, having a higher level increases the chance of finding higher ranked skills, so as you progress through the story, you should get even better chances at finding decent stuff.
Also, remember to always use skill evolution if you happento have a skill you like. Skill rank is extremely important.
